do we need an i693 to file k1 k2 aos? I thought all we needed was the vaccination worksheet from st.lukes ,the rest of the medical info I thought was in the sealed envelope?

Many have submitted the DS-3025 from the overseas medical exam with the AOS package and many do the 693. It is a tough choice. Unless you know how to read the DS-3025 and know which vaccinations are required or still pending, my suggestion is to do the I-693 transcription. Many have had RFEs after submitting only to 3025. Its a challenge finding a physician that will do only the transcribing to a 693. Most will insist on another full blown medical exam.

You can try just by sending in the vaccination records from St. Luke's aka DS 3025. Like the other poster have stated, some get RFE but some are ok. Technically speaking for the K1, if your medical overseas is within a year then you should only pass the DS 3025 and no need for the I-693.

I was on a K1 visa and when I filed for my AOS I only sent my DS 3025 and did not have any problem with it. I was thinking of getting the I693 IF AND ONLY IF it was requested in an RFE. It was per instruction from the USCIS form I485 that if the medical was done within a year before filing AOS then no need for the I693.

You do not need the I-693 unless you had a medical issue that was noted during the visa process (per USCIS when I contacted them). Most only need the DS-3025, but there are some adjudicators that don't seem to be aware of this with the K-1 visa so I have suggested to others to attach a copy of page 4, item 7c of the I-485 instructions highlighted as this notes only the vaccination record (DS-3025) is required. No one has any issues (RFE) submitting the DS-3025 that have done this.

I used the DS-3025 forms for myself and my two K2's. No RFE so far.
