Hello,

I came to the US finally and got married (K1 visa), hooray!

Now I am putting together the stuff to adjust my status.

I am unclear on what is required of me in regards to my medical report.

On the USCIS instructions http://www.uscis.gov/sites/default/files/files/form/i-485instr.pdf it says I only have to submit a "vaccines supplement" if my required vaccinations have been completed. What is this?

Is this something I have to get from a doctor? Or do I just send the Knightsbridge report and a printout from my GP showing my updated vaccinations, or is this insufficient?

When you had your medical in your home country for the k-1 visa the doctor should have given you a copy of your vaccination sheet. That is what they are referring to.

The vaccination supplement they are referring to is the DS-3025 form. If you have a properly completed DS-3025 and your medical exam is less than 1 year old, then you only need to submit a copy of this DS-3025.

Hi, I should clarify

My vaccination report given to me by the knightsbridge doctors does NOT indicate that my vaccines are complete, as I decided to have them done at my GP.

My vaccines are now complete. Which is shown on the printout my GP gave me later.

What I need to know is whether I need to get this certified by another doctor, or whether the printout from my GP will be sufficient.

You will need to see a civil surgeon to have ALL of your vaccinations transcribed to an I-693 form. You will need to submit this I-693 form with your AOS packet.
