Also on the place of marriage -- do I put just the city and state?? And is there an "A -number" on the green card? 0_0

Do I leave the "In Care of Name" blank and fill in the rest?

Do I put my wife's current last name (my last name) or her maiden name??

Yes,put current official name of your wife.

Also if she never used her maiden name in US don't put it anywhere.

Means if she did AOS previously and changed her name in US after marriage then put it in 'other names used' column.If she came on cr1 and already had changed name in her home country then don't put it anywhere.

Its better to write city,state and country.

Whose alien # are you talking about.In case of beneficiary it starts with A and its on green card.You can also find it on old docs of your wife(if she is beneficiary) which she received when came to US or adjusted status.

In case of naturalized citizen its on naturalization certificate and US born citizens don't have alien #. :)
