Hi again!

Regarding form i-693: can i go and see now (i'm in the U.S since last week) a civil surgeon ? Or do i have to wait to be married?

If i can fill it out, what s my family name to put on this form?

So confused.

Do you have some required vaccines to do? If your DS-3025 was properly filled and you had all the required vaccines at the time of your K-1 medical, you may skip the I-693/civil surgeon step in the US. Instead, you can just send a photocopy of your DS-3025 (given at your K-1 medical).

Normally, your original DS-3025 and other medical docs were in the sealed envelope that your handed over at POE. As a K-1, you already had a medical, so if your vaccines were in order at that time (and if it was less than a year ago), you are not required to send a I-693.

About your other question, it depends. Do you want to change your name after marriage? If yes, then you have to fill in ALL the AOS/EAD/AP forms with your new name. That's the name that will appear on your green card and EAD/AP card.

There is a box for "other given names" I believe, that's where you would write your "old" name.
