Need some assistance and guidance on how to proceed.

my situtaion:

i am basing my self to be an automatic US citizen based on Child citizen Act 2000

i was only 17 years old at that time. i have been living on US soil since i was 2years old.

My mother became a US citizen in 1994 but she forgot to do my papers when i was young

since i am at age already i am doing it now.

<My problem is before i even turn in the form and waste hundreds of dollars.

My birth certificate last name is different from my permanent residence card and Passport.

For example:

My birth certificate last name is showing marquez (which is my father last name)

my permanent residence card and Philippines passport is showing: Torres (mother maiden name)

Also i am unable to retrieve a new birth certificate because i dont have anymore family in the philippines

What should i do? or how to proceed to do my application.
