Vaccination Suplement for 200 bucks

I just had my vaccination suplement in Pittsburgh, it cost 200 dollars, after a few questions the doctor said i would need 3 shots: TD, MEASLES MUMPS AND RUBELLA AND THE CHICKENPOX VACCINE, he was very kind and gave us a sealed envenlope and a copy for me.

The form was filled and signed by the doctor.

I have filled all my documents with my maiden name and the vaccination suplement was filled with my married name.

Can I send it that way or should I change all the forms with my married name?

When you are submitting your AOS documents (and EAD or AP), you should use whichever name you will be using legally now that you are married. If you want to take your husband's surname, use that name on your forms. If you want to keep using your maiden name, use that name on your forms.

The marriage certificate that you will be including with your applications will show why your name is different (if you chose to use your husband's surname now).

You should use the same last name on all the forms :)

I think $200 is wa-a-ay too much for a vaccination supplement - I paid that much for the whole medical! :) But I'm glad you're done with this step, congrats! :)

My husband's supplement cost us $50 and that included the Tetanus shot he needed. We called around on the list of CS's in our area though, before we settled on one. We had to drive 90 minutes but it was well worth it. Sounds like you paid waaaaaaaaay to much there.

Oh yes, the 200 bucks included the 3 shots (vaccines)as well as the form!

That's still way too much.
