K1 AOS Immunizations

We are filing for AOS (k-1) and it is requesting immunizations records. His medical was completed and all immunizations completed but all records were taken at POE. How do I receive another copy of the immunizations for the application?

I have one copy vaccinations sheet from the doctor, including my health certificate. The clinic gave me those after I went back to get my medical result. Are you sure he did not have one?

Positive, I was with him. We were only given a sealed white envelope and a stern reminder that isn't to be tampered with or opened.

What doctor did you go to? I had my medicals with Dr Tome Abarca and I was given another envelope with the DS 2025 (or 3025, I can't remember the exact number) which is the vaccine spreadsheet. I took a copy of that form and send it with the AoS

It was a man. The name was something De Carmen.

I know he will need to see a Dr. again to resolve the immunization issue. We are ok with that but is there a specific form we should take to the Dr. with us?

No. The doctor with last name del Carmen will give you that same form after he completes the vaccines. I know several people that have been to that doctor and been given the envelope for your records.

If you are positive that he had all the required vaccinations and they were marked off on a correctly filled out DS-3025, then just make a note that his vaccination records are already in his file/included with the medical documents turned in at his POE.
