Where to put mailing address

I notice on all the AOS documents there is a space for physical address but none for mailing address.

My mail DOES NOT get sent to to my physical address. Only to my PO Box.

What do I do?

Why not put the physical address followed by the PO Box number? For example:

546 Massachusetts St. P.O. Box 66

then the city and state. That is what we did for the company I used to work for as the physical address had no mail delivery.

Thanks Dave. I have a long physical address, so it will not all fit. Is okay to just put the PO box?
