Help !! Wrong date on medical exam form, What should I do ?

Hello ,

I got my medical sealed envelope yesterday and I was supposed to do my medical exam on 01/ 13/ 14 and 03/ 13/ 14 but since I could not get my vaccine done until 03/ 14/ 14, I changed my second appointment to 03/14/14. the civil surgeon performed my physical exam on that day and right after that he signed the form, but the date is 3/ 13/ 14, and also on the dates of exam are like 3/ 11/ 14 and 3/ 13/ 14 and when they had me signed the form , I put the date 3/ 14/ 14, and then they gave me envelope. I noticed all of them on the copy form. The date that civil surgeon signed the form is not the same as the date I signed the form, and so what should I do ? Would that be a big problem with USCIS ? Should I get another medical exam with another doctor ?

Won't be a problem. The only real significance of the dates is that they must be within a year of your adjustment of status. A day here or there is no issue.

What about vaccine record ?

My MMR vaccine record should be 06/ 08/ 10 and 3/13/14 , But they make a mistake by filling out 06/ 08/ 10, 08/06/ 10 and 3/ 13 / 14 like three times. I only got MMR vaccine two times.

As long as it documents that you have the required vaccinations then that is all that matters.
