Just sign where it asks you to sign. You might get RFE for signing in the wrong spot ( I have heard people got RFE for it). You need to send the original letter of intent to USCIS. You can notarize another letter of intent at your bank for free but I dont think it is necessary to do so. Letter of intent just needs the original signature so you dont have to notarize it.

You can copy the envelope of DHL and send it as a proof of relationship. Pictures need to label the date, and place of taking and who is in the picture. I bought a little stick note to do that job.

Birth certificate and I-134 are original. I am sure they will not lose it because they will request at interview date and they will give it back right away after comparison between original and copy.

Please Double Check your package, such as signature and date. Make a copy of entire package for your own safety.

Ombudsman sent a follow up to USCIS on 03/18/2015.

I-485 approved on 03/31/2015 ( we ordered your card).

Welcome notice was mailed on 04/01/2015.

Received Welcome Notice on 04/08/2015.

Card was mailed to me/picked up by USPS on 04/08/2015.

Received GC on 4/10/2015.

Prediction 04/08/2015

You don't need to copy every page of your passport. Just the biograhpical page and your Vietnam Visa stamps when you 2 were together. There is no point of sending copies of blank visa pages. Cover let
