Do both my wife and I need to send this form for AOS package or just my wife?

Also for the applicant employment of last 5 years section. For the first part i put "In process of AOS" from 01/2014 to present time. and then I list her last 5 years of work before that. Is that ok since she ins't currently allowed to work?

She is also changing her name and putting my last name as hers now. For Family name at the top of the form I put my name then her first and middle, but in the example form at the very bottom they have the persons name before they were married. Should I stay consistent and put her name or just do what the example form shows. Just for the record the example form has both the new name and the old name.

Only the foreign spouse is filing for AOS, so only they need the G-325a, as stated in the instructions for the I-486 and the AOS guide here on VJ.

Why would you list her employment as "In process of AOS"? That is not a job/employment. You would list that she is unemployed from the last date she worked until the present.

The example form has a mistake there. It should also be her new married name used there as well.
