Generally it's six months from your medical.

I had one printed on my visa. It states expiration date July 21, 2014. My medical was done on December 2013. So it means, I could wait up to 6 months before I enter to the US but not after July 21, as far as my knowledge.

when yer lass receives the passport back,

there's a visa stamp inside.

Generally speaking, the duration on the two dates represents 6 months,

with the first date being on or around the date of the medical exam.

She could enter on the last day printed there, and be allowed in. I wouldn't risk it, trying instead for the last week at a minimum.

You can wait until the expiration date printed in your visa. You MUST enter the US by that date.

ASAP. You can fly even after a day once you got it. But do make sure all the necessary flight-out (of home country) documents aside from the visa is secured.
