Rob & Monika, VeeNDee, sorry I am a novice in this, I got my passport and the brown envelope back last week, and will be moving soon, I have tried following the timeline on the next steps, but some steps looks complicated especially to someone like me... but VisaJourney forum has been helpful up till now. I appreciate and thank everyone that contributes to this forum.

Rob & Monika, you said ''It depends on what the DS-3025 says, If it says you will require more vaccinations, then you will need to take care of that and get the I-693 filled out as well'' how do I figure this out, is it a general form I have to fill out? or will I be getting a separate DS-3025??

On the form I had, it stated that either I had had the vaccinations or that there was a waiver. I had a specific waiver for one shot due to health issues, and a blanket waiver for the rest. In other words I didn't need to get them. That sufficed for my AOS. You can just check the form for the blanket waiver checkmark.

By the way I got the copy of my vaccination form when at my medical - I asked for it at the office but they were planning to give it to me anyway (along with an extra copy of the xray dvd for some reason that I've never needed). I had my medical at Dr. Cheema.

I have a copy of my vaccinations and the xray disc, so do I still need to fill out the DS 3025 form, or should I make a copy of my vaccination records and include it?

If the copy of the vaccinations is the one that was filled out at the medical...that should be the DS 3025 that is needed.
