I am trying to figure out the vaccination record for the AOS. My hubby has a orange foldable card with a record of his vaccinations done in Canada. Can this form be used for the AOS? Does anyone know the card I am speaking of? It says "Immunization record" on the cover. I am thinking of having my hubby call the civil surgeon and mail a copy of his physical to the US.

A Civil Surgeon will have to transcribe the information from the orange card onto the USCIS Vaccination supplement... That's all you should need..

Or if he has a copy of the DS-3025 vaccination worksheet from his physical in Canada, that will work too. If you're a risk taker, you can risk sending in the DS-3025 for AOS without having it transcribed to the I-693A. Some have been successful and not received RFE's. I haven't received an RFE for it (yet).

I took my vaccination worksheet to the Civil Surgeon to transfer the information to the vaccination supplement. I also took along the original records of my immunizations, including the orange/yellow folder. The office I went to complained about the legibility of the panel physician's handwriting (heheh go figure, a doctor with poor handwriting!) and ended up using my original records, including the immunization folder, instead of the worksheet. You shouldn't have a problem if the information in it is complete and up to date.
