The interview itself was actually very straightforward. We arrived and hour early, Los Angeles Building, was called in an hour and forty five minutes later. Very nice woman asked us how we met, who was our cosponsor (mom) and general chitchat about vacations and why we didn't have a huge wedding. Then the bombshell the Civil Surgeon did not complete our I-693 he left quite a few boxes unchecked so it looks like none of the tests were performed. They actually tried to call a couple times but no one answered so now we have to wait until we can contact them to fix this as we have another rfe for this document to be corrected or worse he has to go through all the shots again which he doesn't want to do.

We're both furious.

So heed my warning anyone trying to get medical done from Jeffery Adam Ostriker from Costal Medical Corp dont. We thought something was off after we first saw him but now that we have this fix it letter...

It's just frustrating.

Sorry to hear about your troubles. Maybe this can help others check over their papers and/or avoid that civil surgeon. You should have him redo it for free.

Hello there,

We had exact same issue. We were asked in the interview why the civil surgon did not fill out some of the lab results. The worst part it the officer said she will call the doc but after 2 weeks we got a Notice of intent to deny unless we submit completed medical document. :(((

Please cross check all medical documents

Wait... I think the problem is rather that his medical in Australia was well over a year ago and when you got the I-693 it was only to transcribe the vaccinations so the medical exam section including the test results would have been intentionally left blank, so you will likely need a full I-693 to be completed now.

I checked them over and thought everything was good to go but unfortunately no. We thought about going back to have them redone there but are scared to return to them, this could have been a biggie (still unsure if it might be) but hopefully not. We made another appointment at a closer office with an appointment on Monday.

Does anyone have any links to what a properly filled out form should look like for those who may face the same issue as we did?

I'll keep you updated and wish luck to those of you going through the same thing

Wait... I think the problem is rather that his medical in Australia was well over a year ago and when you got the I-693 it was only to transcribe the vaccinations so the medical exam section including the test results would have been intentionally left blank, so you will likely need a full I-693 to be completed now.

We were told we needed to get the whole thing so last year he got the full med exam but since a year has passed since that one... I believe we need to get the full one again which is what the agent told us. Luckily I think he wont have to get the vaccinations again but is not happy about having to have another flu shot. When we sent the package in, we were within the time limit so it's our bad luck that it took a bit longer (them misplacing our birth certificate/marriage license, further info on co-sponsor - and then having the package sit on someone's desk for over 30 days until we called and next day it moved). The interviewer seemed as if she wanted to approve us right then but alas... here we still are.

Update. Went to a new place showed them the old I-693 and they were surprised to see all of the mistakes on it. Showed us what would and should be checked off, did the couple of test they needed, we go back Monday for results and paperwork. Hopefully we'll have Green card on hand soon. Just a bit discouraging since so many with the same dates as ours went through so quickly.
