My wife has changed her name to my family name. She had entered last name as my family name. If your wife has changed her name officially including passport she must enter her last name which is in her passport. It is understood that after marriage if spouse change their name.

Good Luck

But in the box says "For wife, give maiden name", shouldn't I put her parent's last name?

It is clearly asking for maiden name, so in that section you put your wife's previous last name, yes, her last name at birth.

The maiden name is the surname that a married woman used from birth, prior to its being legally changed at marriage. The term maiden name is hardly ever applied to the change of family names by men, or by either sex other than in connection with marriage.

Maiden name is name at birth, which may or may not be her parent's last name.
