Regarding medical, I had two minor surgeries. Unfortunately I do not have any paperworks with me regarding these surgeries. Is it a must to have the documents?

Well, the question is applicable as the beneficiary can have an A number (I did). So, if the beneficiary doesn't have one, then write 'none'.

Checking in NVC received case on March 17th.

We called the first time a week after NOA2 and the rep told us to call again in 8 weeks. We called again a week later, found out case was received and the (possibly rather optimistic) rep told us to call again in two weeks for case #.

Well, medical records are needed. Can you not go back to the hospital where you had it and ask for some kind of documentation or a letter from the doctor?

I've run into a problem and I hope you all can help me. I was filling out DS260 and under the present and previous addresses section it asks "From date". I'm not too sure what exactly its asking about and after I finish the section it gives an error saying "There is a gap greater than a month in the dates you have provided. Please include ALL addresses where you have lived since the date you turned 16 years old. The National Visa Center may ask for additional information regarding gaps in residency, which will result in a delay in scheduling your visa interview" . Is this error because of the P.O. Box address I provided as the present address. (the mailing system in Saudi Arabia is such that only P.O. Boxes are provided in place of actual mailing addresses).

As the error states, there can't be any gaps. So, all dates have to be back-to-back.

05/2000 - 06/2012

No more gap! 06/2012 is back-to-back with 06/2012.

For me it was within the same day. I'm waiting to be able to pay now, because I have my triangle of doom, but it won't allow me to do the transaction yet. I'm just checking every half our or so. (I'm waiting for the IV though.)

Question guys. If i am using my wifes mother as a sponsor for the AOS, do i even need my wives taxes for 2013 ready for the NVC stage?

Thanks for the swift response. That sucks. I doubt if i can get the medical record for the first surgery as it was done long back. Unless it was a major surgery they shouldnt ask for medical records i feel.

But here's the problem, I understand everything you explained however its still giving me the error and since it might be easier to understand the problem with a screenshot I have attached them to give you a little bit of an idea. I have been living at my present address since I was three years old (Jan 1992) but I left for my college education from Nov 2006 to March 2012 so those dates are confirmed. I don't understand where exactly there is a gap of more than a month.

Its because you put 2004 you weren't 16 until 2005. Correct that, It's where you been since 2005 and go from there.

so it should be

06/2012 - 03/2014 college

01/2005 - 06/2012 Home address

They are very stern on the year you turned 16.
