All we did was have my wife's DS-3025 vaccination information transfered by a Civil Surgeon from the DS-3025 to the I-693A vaccination supplement. The civil surgeon provided my wife with the original I-693A in a sealed envelope, which we attached to the original AOS application, and a copy for our records.

My understanding is that for a K1 visa holder, so long as the AOS application is filed within one year of the original medical examination which took place in the K1 holder's home country, all that is needed is the I-693A.

Regarding the HIV test, I believe that is required in order to obtain the K1 visa, and should have also been done as part of the original medical exam in the K1 holder's home country. The doctor performing the original medical exam in my wife's home country provided us with a list of all tests and all vaccinations. I am sure all of this stuff was in the sealed envelope given to the immigration officer upon first arrival in the USA.

12-22-05 I-129F mailed to CSC

12-27-05 USCIS receives I-129f

01-03-06 NOA1 by mail

03-14-06 NOA2 Case Approved

03-21-06 NVC sent case to BKK

03-27-06 BKK Embassy email "no record of my case"

03-27-06 NVC approval letter rcvd

03-30-06 BKK Embassy confirms case by email

03-31-06 Fiancee Medical passed

04-07-06 Packet 3 rcvd

04-20-06 Packet 4 rcvd

05-26-06 Interview 730am VISA APPROVED!!!

05-30-06 Pick up Visa 3pm

Can you tell me how this works if you are missing vaccinations? I never got an I-693 form but do have DS-3025.

*Came to see me Thanksgiving week in Nov. 2006

*K1 Interview - 2/2/07

*K1 Visa received - 2/11/07

*Date of US Entry (POE Chicago)- 3/5/07

*Wedding/Marriage - 3/17/07

AOS (My case was expedited due to husband going to Iraq):

*Filed for AOS - 4/20/07

*Found out in the beginning of June that husband is going to Iraq

*NOA for I-485 - 6/11/07

*Made Infopass appointment to get case expedited due to deployment (Infopass appt 6/12/07)

*Biometrics - 7/7/07

*Interview date - 7/11/07

*I-485 Aprroval date- 7/11/07

*Green Card Received- 7/19/07

Removal of Conditions:

*Filed petition to remove conditions on 6/9/09

*NOA- 6/15/09

*Biometrics Appt. in Birmingham - 8/6/09

*Lifting of Conditions Approval Date - 10/22/09

*Waiting for Green Card!

Had our daughter on 4/4/08 and have another baby due 11/19/09!!!

The civil surgeon transcribing your vaccination information from your DS vaccination form to the I-693A will tell you if you are missing vaccinations. If you are, they can give them to you but you'll have to pay extra for them.

2007-05-10: Biometrics appointment (both - Biometrics review)

2007-06-05: AOS interview letter date

2007-06-13: AOS interview letter received in mail

2007-07-03: EAD card production ordered

2007-07-07: EAD card received! (yay!)

2007-08-23: AOS interview (Documents / Interview review)

2007-08-23: Green card production ordered!!!

2007-08-24: Welcome notice mailed!

2007-08-27: Green card production ordered again... ?

2007-08-28: Welcome notice received!

2007-09-01: Green card received!

Done with USCIS until May 23, 2009!

I suggest you call around, the prices can vary by a lot. We ended up paying $70 for the I693A and the tetanus shot she needed. They did want to have her get another xray so I whipped out the one from her medical and they accepted that. I thought they did the HIV at the medical but may be wrong but don't believe it should be mandatory for this form, this is the vaccination supplemental! Good luck :thumbs:
