Don't have Copy of Medical report but require vaccination verification

Timeline

Hi All,

We are now married and I am trying to get the AOS completed and EAD/AP.

I did not have my vaccination records at my london medical.

However I am looking to get my shots done today and schedule an appointment with a civil surgeon for verification.

I do not have the I-693 that was completed in London.

I do however have the vaccination part.

I am confused here, do i just need to get the vaccination parts completed and sent in with my AOS. Immigration have my medical report.

Any help is appreciated. What do i do if i don't have the full completed I-693 from London.

I don't think anyone gets the I-693 just the 3025 i think it is. Take that along to the civil surgeon after you get the shots done you need and they should transcribe it on to an I-693, I believe they give it to you in a sealed envelope to be sent with your aoS package. Think thats how it ahppens anyway but I have not had to do it myself, let us know what happens.

I-693 is the medical form for a civil surgeon in the US.

Medicals abroad are done by panel physicians and the form numbers are DS-2053, DS-3024, DS-3025 and DS-3026. See examples here http://www.visajourney.com/downloads/Medical_Forms.pdf They are on file and will be matched up with your AOS application. Knightsbridge will have given you a photocopy of the vaccination page. If you took any immunization records to your exam, they would have noted on that form what shots you had already proven. Do you have the Immunization for DS-3025? Are their any shots listed or record of Varicella (chickenpox) history recorded on it? It would be marked VH in the column by varicella. That would help determine what shots you need to get at Walgreens.

After you get the shots you need, a civil surgeon will have to review them and sign off on an I-693. You do not need a full medical exam. The civil surgeon only has to complete the vaccination portion. The problem is, many civil surgeons will insist on a full medical exam or a TB skin test. NO--not required. You have already done a medical exam including being cleared of TB. Keep calling and asking prices for immunization clearance only before you make an appointment. If they don't sound like they understand the process for a K1, then keep calling. The form I-693 instructions can be downloaded. Read what they say about K1s who have already had a medical exam so you know what to do.

I have the DS-3025.

The document has VH- Varicella, V k-1 Visa for (Td, MMR and Flu). On my military medical docs my immunizations are over 10 years old.

I will go ahead and get the shots for the 4 items above.

Then proceed with the registered Civil Surgeon on 1111 Medical plaza drive. Dr J. Rivera. The 4 shots are what they also quoted I needed.

Ill take the DS-3025 with me and the shot records.

Then onto the full AOS docs.

Do you know if I could get the Biometrics done quickly as I have to apply for EAD and AP to get back to work soonest.....I work offshore Angola.

The application for AOS and EAD kick off the biometric appointment assignment. The appt date should arrive within 30 days. We did an early walk-in (after appt letter but before actual appt date) each time. However my neighbor tried that two weeks ago and the lady there refused. You will be using that same office on Hwy 290. Kind of a long drive to get turned away. Not sure why the hard nosed reception because we have done 3 walk-in appts now with that office and they couldn't have been nicer.

Ok...this is frustrating...

I get all my shots done today...arrive at the Civil Surgeon after talking with them about vaccination verification....

....the CS won't sign the I-693 without undergoing the full medical.

I told them my medical has already been carried out and it is only for vaccinations.......wasted trip.

Now I am concerned as to who will carry this out.....If any one is in the Houston area who knows of a CS who understands the K-1 process please feel free to let me know......I guess a few phone calls tomorrow........The I-693 even has a statement for vaccinations only on it!!!

I know I was advised this would be the case in this thread...however I had discussed this with the administrator on the desk beforehand.

Any advice is appreciated...
