Please tell me if the vaccination sheet is Complete

Hi you guys,

My husband did his medical exam overseas, and the Dr never gave him a copy of his DS-3025, but we contacted her afterwards and she basically rewrote it.

She didn't list any dates on the previous vaccines he's taken, is that going to be a problem?

Also wrote 'Not age appropriate' next to MMR, is that right?

Please look at the attached sheet and let me know what should be done.

Unless your husband was born before 1957, then the not age appropriate by MMR is not right. Meningococcal is not marked at all. I would say your vaccine supplement is incomplete.

thanks, no my husband is only 30 years old. I am guessing the Dr really sucked, even though she is a civil surgeon, she gave him the Hib vaccine which is not required for healthy adults.

It's just a mess

I do have another question though, during the time of the exam the flu season didn't start, but now it did, should I have him see a civil surgeon to get it and have it transcribed into the form or it's not necessary?

You will need to see a civil surgeon anyways since your DS-3025 is not properly completed. You will need to submit the I-693, filled out by the civil surgeon, with the AOS application.
