AOS Vaccinations

Hi all, its been a while since I've been on here, I'm just about ready to send in my AOS packet, and understandably im nervous.. here's why. I had my medical back in London in August 2013, who gave me my FIRST Hepatitis B vaccine, and told me I needed my second one either in 4 weeks or when I got to the U.S. I just had my second one today (Feb 21st), am I required to have a third one? The doctor who gave me the vaccine wasn't very clear, he just signed the paper to show I had it done and sent me on my way.. If I do require a third one, and I send my AOS packet in, will they deny my application? Also, I'm confused on what I need to send in for the medical side of things, do they need all the paperwork the doctors gave me? (Vaccination chart and receipt of the vaccinations, proof that I had medical)? And do I need to send in any other medical forms like the I-693?

You are only required to have the first shot in a series for AOS. The only requirement for your AOS is either a fully complete DS-3025 or a completed I-693 filled out by a civil surgeon.

at the bottom of my DS-3025 it is checked as "Vaccine History Incomplete, Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate. I take it i need to find myself a civil surgeon?

You didn't read the link? It says part of being an acceptable form it must say--

3. RESULTS section filled in with "incomplete" and "may be eligible for blanket waiver" ticked or "complete" (but complete is rare).

So why the Hep jabs? Are you under 19 years of age?

Also, your medical results were in the envelope turned in at POE. They are supposed to get matched up with your AOS file. Sometimes USCIS loses them, but nothing you can do about that. If your vaccination form is properly done, you can send a photocopy and your done. If you were missing the shots needed, as detailed in that link, then see a civil surgeon. Study the link explanation and determine if you are complete already.

And tell us your age at medical exam concerning need for Hep B.

i turned 18 one month before my medical exam, thats why I needed the jabs. I read it but it didn't stick in my brain, I just read it over again and understand it now. I know I may seem young and over my head in all of this, but I matured at a much younger age lol.

EDIT: None of my other shots were missed, I wasn't required to have the Varicella as I had chickenpox as a child.. the only ones i was missing was Hepatitis B, I had one on the date of my medical and the second today here in the U.S.. I had it done by a military doctor who of course couldn't sign the DS-3025 saying that I had the second jab, so if I send in the signed and dated official note he gave me today (with the DS-3025), will they accept that as proof of my second jab?

I saw your husband is active duty, I don't know if you know but include his military orders with that.

You only send in the DS-3025. Do not send in other vaccination records or notes about receiving other vaccinations. Those are for your records and are not needed for AOS.

Did you also get Flu shot?

Well are you sending AOS soon? We have here immigration officer on post and she told me to include military orders with every package we sent. It helps and make process sometimes faster. My husband is active duty also.

The only reason I asked your age was to know if you needed Hep B. And if you turn 19 before AOS decision comes up for Interview or without interview, then Hep B won't be an issue because 19 year olds don't require Hep B.

So your DS-3025 has all this on the sheet. Other doctor notes don't count. Must be all on DS-3025.

DTP or Tdap or Td given--some version of that dated within last 10 years.

Hep B (one jab) with a column marked "insufficient time interval"

MMR (2 doses) or (one dose with a column marked "insufficient time interval")

Varicella marked VH in a column

Flu jab given, marked, dated

Results- incomplete and may be eligible for blanket waiver

Doctor signed and dated.

That should do it. No civil surgeon unless your medical papers turned over happen to get lost. Send a photocopy of that DS-3025 vaccination sheet with your AOS.

Yes we're mailing it out tomorrow, we have one here on Fort Benning too who we went to see. She said I won't need them with the medical side but to send them with the overall package to be safe.
