Vaccination suplement

Im almost done with gathering all my documents for AOS, the instruccions tell us that if I am a K1 visa holder and had a medical examination within the past year as required for nonimmigrant fiance visa I only need to submit a vaccination suplement (not the entire medical report) with my AOS.

When i had my medical examination in Ciudad Juarez doctor told me I wouldnt require any vaccinations and they gave me a sealed envelope which i took to the Port of entry (HOUSTON) and I gave it to the oficial.

My question is now HOW CAN I GET THE VACCINATION SUPLEMENT??

Find a civil servant in your area. I suggest you call around, the fees can vary by a lot.

Thanks for your reply!! The vaccination suplement means they will put on me any vaccine or it is just a form that the doctor will fill and give it to me? Im wondering because in Ciudad Juarez doctor told me i wouldnt requiere any.

In CDJ he probably told you that you did not need them because most K1 applicants in CDJ don't get them there - they don't "honor" vaccination records that applicants bring to them and they overcharge.

The vaccinations that you need are based on age and what illnesses you had or did not have as a child. A civil surgeon in the US will either have to give you a blood test to determine what you need, or will just give you all of them and charge you accordingly. Whether you need vaccinations or not, you still need the I-693 form filled out by a civil surgeon.

The London P3 medical instructions say that any vaccination record from the K1 medical will be "usable for AOS without any further work." Is that true? What does that mean? I thought they took all those records away at POE, never to be seen again. Can we have the embassy doctor fill out the AOS vaccination supplement?

And another question -

The vaccination supplement is just a couple lines on the I-693, yes? Assuming an AOS applicant is exempt from the physical because he had one for his K1, does the civil surgeon just leave the rest of the form blank?

Nevermind, I've figured this much out. Is the I-693a available anywhere online? USCIS website just seems to have I-693.

The I-693a is a form used by the civil surgeons. I don't have any link to it. If I can find the copy the civil surgeon made of Claudeth's before he sealed the original in an envelope we mailed to the lock box I will try to scan it for you. I just can't remember what we did with that stuff.

Oh, don't work too hard on this! It's mostly for my own curiosity. As long as the CS can come up with the form, I suppose it doesn't matter if I have a copy beforehand.
