I am feeling out the DS-260, there are couple of questions needing help with. One is about if my spouse had all the vaccinations in accordance to US law..., what should I answer? She had vaccinations when she was a baby, but don't know if they are in accordance with US law.

Second about social security card, should I say yes I want her to receive one or no?

There is also the question about work when she gets to US she will work or no? what is the best answer.

I answered no to the vaccination question... had to give an explanation either way so I said any required vaccinations will be administered at the medical. Yes you do want her to receive a social security card. This will enable her to work. Not sure about the last question, maybe someone else can help you there. I am a homemaker and said that is what I will be when I get there, but if your spouse is qualified to work in an area and she wishes to pursue work then I would answer Yes

She had vaccinations when she was a baby, but don't know if they are in accordance with US law. - unless she has proof of the vaccinations required by US immigration answer no and that she will recieve all required vaccinations at the medical.
Second about social security card, should I say yes I want her to receive one or no? - of course she wants one! Everyone in the USA needs an SSN. But this doesn't mean she'll automatically get one as only about 10% of people do. Most likely she'll have to go to an SSA office and apply for it herself.
There is also the question about work when she gets to US she will work or no? what is the best answer. - will she work? will you decide that when she gets here? What is the truth? Answer the question truthfully.

I put down I would work in retail, which is what I was doing and what I am now doing in the USA. But you don't have to work and you don't have to know what you're going to be doing. To be determined is a suitable answer as it homemaker. The only time any issues come from this question is if whatever job they want to do, requires specific training or certification in the USA, in which case you'll have to seek said training and certification.
