Hi All - I need help - I am a K1 fiancé visa applicant and doing my AOS. I am unable to find a doctor in NJ who could certify my vaccination records since I already had my medical from Pakistan and its still valid so now what only I need is the first page of I 693 form and the vaccination record work sheet.

Here in NJ - all the surgeons in the USCIS list want me to do my medical again and no one is aware about certifying vaccinations only - can any one help me with this please.

I don't know why ever I post and I never get any responses - I hope I do it correctly. Help is appreciated.

I've heard of this one before! I guess it is better for them to charge for the whole medical, than for a few more vaccinations.
Below, you can find all information as stated in http://www.uscis.gov. It can't be any more official. Maybe, you should print out this info and have it with you, when going to the surgeon.

b. You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

Bring vaccination record, or get immigrations related vaccinations and get the record transfered to the I-693 by a US civil surgeon.

You only need to submit the DS3025 IF it was filled out correctly.

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

The key term in number 2 is "properly completed." We did not go to a CS and Joan's Green Card is in hand with no RFE!

Yes I am a K1 - I am ready with my application - just a quick question that do I need to attached additional pictures for my EAD and parole document or just two pictures altogether.
