My wife changed her last name to mine when we got married. She didn't have one before. Her birth certificate just has her first name. Last name is blank. Her afghan identification paper and our marriage certificate have the new name on it. The passport though has the name written as (first name)(new last name) then under surname it says D/O (her father's first name). So im not exactly sure what to put for surname on the DS-261. Should it be just the new last name or do I have to include the D/O blurb too? Thanks for the help

I would not be wrong, but when you complete forms DS-261 & DS-260 first and last name are automatically wrote.

then please follow the way i suggested to you. you can tell your wife to take the passport to the passport department. They either correct the passport according to new name based on the identification card, or they will revoke this and issue a new one. make sure you keep a copy of the old one in case they issue her a new one and keep the old one.

The alternative is to wait until your wife either gets a new passport or fixes the passport. it should not take more than 2 or 3 days. then you can fill out the form 260 and 261 accordingly.
