Civil Surgeons Transcribing DS-3025 to I-693A (Texas)

1 I would assume that they have that form, it is not on the USCIS site.

Is now a US Citizen immigration completed Jan 12, 2012.

Thanks, and I guess it now dawns on me that this would be a lot easier if actually done here (in Thailand where the Medical was done in the first place).

Is this possible, or must the transcription be done in the USA?

Yes, the civil surgeon will provide the form for you. Here are the Texas Civil Surgeons.

The transcription must be done here, foreign clinics do not have the I-693.

The CS in Tx will transfer it from the DS3025 which you hopefully have.

Get a copy for yourself in case it gets ' lost' during AOS.

They like it sealed I heard so get your copy from the CS.

If they insists on a complete medical do not listen and find a CS that knows

the regulations and cooperates. Shop around for fee for the transfer, they vary from

$10-$50.

That would be consistent with the government beauracracy. One would think some one, some day, may allow us to have one form for multuiple uses.
