about medical test, I-693

hi everybody,

I got the vaccine shot before the K-1 visa interview, and later they gave me the Medical Examination Record, the vaccination documentation worksheet, a Sealing original envelope of Vaccination Form, and the International Certificate of Vaccination or Prophylaxis.

I'm applying for the AOS now, and I checked they want the form I-693, but I didn't find a civil surgeon to have another vaccine shot. Can I just send all of these stuffs above to USCIS? or do I need to find a civil surgeon to get the form I-693?

PS. Can I do go to see the civil surgeon later, and bring the form I-693 with me on the green card interview day? I found someone sent the form I-693, but they still received RFE for Medical, since the USCIS lose it, so they have to go to civil doctor again.

A Civil Surgeon is the only person who can fill out a I-693. If you are missing vaccines on your foreign medical then you need to see a Civil Surgeon.

How can you not find one? Use the civil surgeon locator at uscis.gov

Thank you! I only had one vaccine shot at my home, but the doctor at my home told me I need have the second shot which is called booster shot after one month later, but so far I didn't get the booster shot. So do I have to get the booster shot when I apply for the AOS?
