Name change at age 12. Original birth and batism vertificates DOUBTS! HELP!!!

I got my packet 3 a day ago. It says that if my birth certificate was corrected, I need to show an original copy of my birth certificate, batism certificate or any other document to support my identity. My name was changed when I was around 12 as my father was willing to give me his last name at last. So my first last name is different now. I have an updated birth certificate, but I don't know if I might be able to get a copy of the original birth certificate. I can get a batism certificate, which will show my name prior to the update done by my father.

I'm stressed about that because I don't know if I could get a copy of my original birth certificate.

What is your advice? Will this situation represent an issue in my visa issuance?
