G-325A Questions (Green Card)

Hi,

I am starting the process of filling out the green card papers for my wife and I have a couple of questions about the G-325A form.

1. On the box at the bottom of the form, should I choose Naturalization or Status as Permanent Resident? It seems like I would choose Permanent Resident, but the example form on this site says Naturalization.

2. My wife is from Japan and she needs to write her name in her native alphabet. She is changing her last name to my last name. Does this mean that she writes her new last name or her maiden name?

1) that is an error on the example form, Page 1 is correct Change Status to Permanent Resident.

2) The example form is from the perspective of changing name. In my case, my fiance is Chinese, we probably will put maiden name and in brackets () after, my family name.
