Document ID and issue for us were proof of citizenship.

My wife used her Passport as the document (passort ID number from the biological page of passport) and the document issue was when the passport was issued. (to the best of my knowledge)

DS-260 is only part of the package sent and to the best of my knowledge, yes, birth certificate and police clearange and such things are needed, as least my wife from Philippines needs those things for our CR-1 package.

hello mich after you pay the iv bill $230 you print the document cover sheet and send to nvc together with the civil documents the nso birth certificate and photocopy, nso marriage contract and photocopy, photocopy of bio page of passport, 2 pcs photo passport size with white background and your name, bdate and nvc case number written at the back, nbi clearance the green form (travel usa purpose) and mine i just send also my police clearance. then once its paid the ds260 is available for you to fill up, for me its hard cause i have lots places i lived since i was 16 and lots of company i worked for the last ten years. review it many times before you submit cause once its submitted its hard to correct it.

1. Previous addresses - Do you start with previous address other than your current address or address from the age of 16?

2. Employer - Do you start from your current employer and the go on backwards? And are they asking employment in US or from your current country and for last 10 years or all employers?

3. Education - Do I enter my latest education and then go backward?

Everything is latest [most current] to least current. High school and up for education, all employers for last 10 years, and addresses from age 16 to current but in reverse order.

No questions about unlawful presence, overstays, previous visas, last 5 visits or any removal orders?

Interesting. I wonder why?

I think that you get these questions based on your answers earlier on. So if you said that you have been in the US before then you get the last 5 visits question and the removal questions. If you answer "no" to have you been in the US before you just don't get the questions.

Also, from what I have read if you have "current employment" you don't get the "previous employment" questions unless you are from certain countries.
