Civil Surgeon get me confused.Please Help (merged)

@Jay Kay but I did all my medical for my k1and they told me it is gonna expire within a year but the embassy did not give me a photocopy of it how do I prove to the surgeon here that I already had my medical?who do I contact for the DS-3025?I called a surgeon here just to book a appointment is $150 before paying for the medical help me

A copy of the DS-3025 would have been given to you at the K-1 medical. Did you receive vaccinations at the K-1 medical? Do you know which ones? If you know you had all the required vaccinations then, you should still be ok without a copy of the DS-3025. It will be in your file along with all the other documents that were in the sealed envelope you handed over at your POE.

You do not need a new medical and should not need proof of your medical results, which you do not have. If you really want to go to a civil surgeon before your interview, then call around to find one that will not insist on a whole new medical. If you have any records at all of your vaccination history, then take those with you.
