K1 Visa to I-485 (vaccination supplement)

All:

If you are applying for the adjustment of Status and your medical was done for the K-1 Visa on August 2013. We are still within the time frame of 1 year and don't need to go for the medical. However proof of evidence is asking for "you only need to submit a vaccination supplement." We don't have this as everything was given to the Immigration officer upon arrival.

Do we need to worry about this? Please help as I am sending the application today.

You can obtain the form on this USCIS website. My wife had her medical in August 2013 as well and they had given her the paper but if you lost it you can print it out. You can have a civil surgeon just fill out the vaccination portion of form i693. Have them seal it in an envelope and give you a copy of it as well. This is what we had done and we havent had any problems. We did have to pay for 1 vaccination and the $35 office visit fee at the clinic we went to but it was a whole lot better than having the exam again. You may have to fight with the clinic some because they will try to have you do the exam again (at least for my wife they did) but we had proof that she didnt need it again etc and they just did the vaccination part of it without a problem. Timeline

If your vaccination record from your K1 medical is complete (you should have been given a copy so look through your paperwork as it should be there) then you only need to submit that and you can skip the I693 all together.

Can someone give me a sample of you this page looks like? We found something which just gives the name of the Doctor and when the test was completed. It doesn't say as to what was completed.

Secondly... I 693 needs to be filled by the doctor you was performed the tests during the I129F application?

Google search Form DS-3025. This is what the examining doctor gave to my fiance (now husband) the day of his medical. Hope this information helps!
