First, you need to determine if you sister meets the requirements to pass US citizenship to her child. You have not provided enough information to determine if she meets the requirements to pass US citizenship to her child. Read this; http://www.uscis.gov/policymanual/HTML/PolicyManual-Volume12-PartH-Chapter3.html#S-B

If the child was born a US citizen, then your sister would need to file a CRBA to report the birth of a US citizen abroad. Your sister would also want to apply for a US passport so the child can enter the US.
