Married name or single name in I-693 Vaccination Supplement?

Hi everyone, I came here in K1, my vaccination record from my country has my single name on it. I'm a little confused here. What would I use in I-693 Vaccination Supplement for my AOS, my married name or single name? I just want to make it sure before filing my AOS.
