I really didn't want to lie and say it was lost or something, so I just had my husband use the following, and we received confirmation by email 2 days later! I thought people might want to know that variations on the email language suggested in the Timesavers thread appear to work, at least in this case. :)

Dear Sir or Madam:

My name is Michael xx, my visa case number is MTLxxxx.

I do not have my DS-3032 and I would like to appoint my wife as my Agent of choice to receive all correspondence from NVC.

Her name and address are:

All mail from the U.S. Department of State concerning my immigration visa application should be send to the address above. Thank you for your help.

NOA1: 5-10-2006----updated w/ citizenship: 9-25-06----had to call back 10/25, touch 10/26

12/06/06 - Approved!- - - 12/08/06 - Touch---01/25/07 - Touch

12/14/06 - case number assigned

12/25/06 - DS3032 & AOS Fee Bill Mailed (phone system updated 12/27)

12/27/06 - emailed choice of agent; 12/29/06 - received email from NVC confirming choice of agent!

01/01/07 - NVC generated IV Fee Bill (postmarked 1/17 though!)

01/03/07 - returned AoS Fee Bill via Priority Mail (James' shortcut)

01/15/07 - NVC generated AOS package

01/22/07 - received IV Fee Bill - overnighted back to NVC same day

01/27/07 - recieved I864 package; 01/29/07 - overnighted I864 to NVC

01/29/07 - DS230 generated (phone system not updated, email response 2/5/07)

02/05/07 - mailed DS-230 to NVC via express mail

02/20/07 - CASE COMPLETE!!

04/18/07 - INTERVIEW!!!! - APPROVED!!!!

Michael's K-3:

09/28/06 - NOA1

1/25/07 - approved ...NOA2 via snail mail - 1/29/07

03/16/07 - chose not to return packet 3 to Montreal

we also didnt wanted to lie .....so just wrote the mail as given below and they accepted in two days ......

so folks since we are sending the email on the ds-3032 generation date itself... so i suggest its better to write email in this manner

I would like to appoint my wife as my agent of choice to receive all the correspondence from NVC. Her name and address is:

IV & AOS bill payed 08/12/11

NVC receivd AOS & IV package 09/12/11

AOS approved 09/22/11

GOT RFE .... 09/28/2011

now NVC wants to see my original marriage certificate and birth certificate.

Case Complete 10/11/2011 (NO interview in November or Dec@ ABU DHABI consulate )

VISA granted...... 1/24/2012 thank you JESUS

I didn't lie either and the email I sent was similar to yours. Also, I called the USCIS and asked them about my husband sending email choosing me as his agent of choice and was told it was fine to do so. It's too bad USCIS do not let you know that in advance.

I just got the NOA2 I-130 approval by email yesterday 02/27/07. I have a lawyer that is taking care of our case. I was wondering if I also can have my wife email NVC the choice of agent to be our lawyer's firm?Any suggestions would be greatly appreciate it. Also any ideas about the time line between USCIS to NVC and NVC to the consulate? We are getting IR visa.Also I am leaving to visit my wife in 2 weeks and will be gone for a month I like to do everything that I can before I leave. Thanks people.
