immunization question

Ok, so I've had all my shots, a flu shot and I'm about to book for a booster shot I need, but previously I received a signed statement from my doctor that I have received all past shots, however no dates are given except for the date it was printed. All my records are old and handwritten, will the us doctor take the signed statement or do they have to have specific dates from my childhood?

Are you talking about the yellow immunization card for your old records? If that's what you are talking about that will be perfect.

no, I don't have one of those, I think that's a little too far back, I'm 33. I went to my doctor, told him I was moving and needed my immunization records, so I got a letter saying that he certifies that I've had (followed by a list) so and so vaccinations and if there are any concerns contact him, he signed it and the only date on the letter is the date it was printed.

Hmm not sure on that one then. I think him stating you are up to date would be good, but I'm no expert. Have you called the Health Unit to see if they had any records? I got lucky my mother kept my yellow card, I have 2 actually since the older one was really old they started marking new shots in the newer card.

what's the health unit? I still have no idea what a yellow card is.

I know back in 2004 I was able to get my Vaccinations from Saskatchewan! I was 31 then, damn, I turn 41 this yr,lol What ever "authority" in your province may have them and be able to fax u a copy (or scan/email)

At least in Ontario they used to give the parents a card for their own records. The Health Unit in Ontario is who keeps track of immunizations, they don't keep the records for ever but some places tend to keep it longer then others.

Interestingly enough, I went and looked back in the package my doctor had given me and all of the immunization dates are in there, in his chicken scratch handwriting. I just wrote them more legibly and will give it all to the US doctor when I go. Thanks for the help everyone.

Everyone flips out over the immunization thing.
They only want you to have records for immunizations that are age appropriate. Being an adult all you need is a DTAP booster, a flu shot and proof you had chicken pox or received a vaccine. Anyone born before 1980 can verbally declare they had chicken pox as a child.
We had no childhood immunization records for my husband, the Dr. at Medisys only looked to see he had a flu shot (since it was the end of flu season) and a DTAP in the past ten years.
