(((A))) - who's done A so far? What did you ask your bank to provide, just normal bank statements that have the date on them?

(((B))) - What the heck is this garbage? The clowns at the USCIS must be kidding, right? What do they want, ask my HR department to scratch down some text on some sticky papers?

On Question 3, is that her name or my name that I put? It's gotta be her name, right? i know it seems obvious, but I want to make sure.

Then it asks:

"""Name of spouse and children accompanying or following to join person:""" and there are boxes below to fill in for spouse. Do I put her info in that box, even though 2 inches above it's already there???

Question 8: I put in her name again?

Question 11? We are married, so am I supposed to say 100% contributions I intend to make to her?

Also, how long is this good for? If I get one done tomorrow, and her interview is in 4 months, is that considered valid still? Is there an expiration on these things? Will she find out in packet 3 how old the I134 can be?

I have recently completed my I-134. Your bank or fiancial institution should be used to this request and will type a letter for you. Make sure it includes all the information requested. Date account(s) opened, amount deposited for the year, and current balance. It was mailed to me in 3 business days. Call your bank or stop by and talk to the manager.

I asked my HR department to give me an employment verification. I was advised the one online that I could access and print was not sufficient. They were used to this request as well and typed on company letterhead how long I had been with the company, my position and annual salary. It took them about 3 minutes and they handed it to me.

I don't know how long it is good for but i sent one when my fiance had packet 3.5 sent to her and we had to submitt all the docs you are asking questions about. Hope this helps.

I did the bank letter a couple weeks ago. My bank is small, but they took it in stride, and an assistant manager asked what information I wanted the letter to provide and she made me three originals on the spot. Easy!

Your monthly bank statement likely includes a bunch of garbage you don't want to share, such as your $120 month double mocha late habit, so why provide that? It also is not likely to include the date your account opened, nor monthly tally your yearly deposit totals. So just head to the bank and ask for a simple letter with the information your embassy/consulate is requesting.

For the employer letter, same deal. Just ask your boss, supervisor, or whomever is above you qualified for a simple letter giving the information asked for. It makes a brief paragraph for him/her to sign. If you know the information, perhaps even type it up for them and have them print it out if they approve on company letterhead for signing. (I didn't think to at the time, so only had my boss make one original, ask for three. One for you, one for your fiance, and one to turn in along with your I-134 which I did get three notarized originals of.)

The sample letter in the guides here will give you the best answers for all of the I-134 questions.

Ok, just finished everything today and ready to bring it to my sweet heart next week. Hopefully I don't have to re-do it.

Unanswered question: Anyone from Guangzhou consulate know of any expiration of the I-134? Like, how long is it valid for after i get it notarized???

Also, I noticed on the PDF it has an expiration of 04/30/07. If her interview date is not before then, does this mean I have to redo the application?

Notarial birth certificates, marriage certificates and divorce certificates have unlimited validity. Medical reports, notarial single certificates and police certificates are good for one year from the date of issuance.

To answer your question, I got a direct response (via e-mail) from the consulate in Guangzhou, PRC.

Here's what they said:
