A doubt while filling an application

I'm filling out an application for medical insurance. One of the questions is if I have changed my inmigration status in the last 12 months. I wrote: "Conditional residency to permanent residency" ...besides, another question asks the date of change. Do I have to write the date GC was in production?

So, in this case, should I write the date my first GC was issued?

I would answer the question about change in last 12 months as "No" (assuming you got your first card earlier than 12 months ago) and date - if it asks for change in general and not last 12 months - your GC approval date (Resident since on GC).
