Surnames

Enter all surnames as listed in your passport. If only one name is listed in your passport, enter that Surname. (e.g., FERNANDEZ GARCIA)

Does this mean I have to type my middle name and family name in the SURNAMES space?

Did any of you put in your middle name and the middle name of your fiance in the DS160 form?

I just want to make sure I don't make any mistakes before submitting this.

Yes. As far as i know, unlike us filipinos, americans do not usually use their mom's maiden names, their middle names are actually given names so you must include your usc fiance's middle name in 'given name' part.

How about our Filipino middle name? It says in the passport "Panggitnang apelyido." Does this mean that I will put my (Filipino beneficiary) middle name in the Surnames?

And my fiance's (American petitioner) middle name should be in the Give Names?

Agree with hank_. Ang Panggitnang apelyido ay nasa given name, hindi sa surname.hope you dont get confused though when i was also inputting mine, i got confused at that part lol.

So, let say the full name is Juan dela Cruz dela Paz would be:

Surnames: dela Paz

Given Names: Juan dela Cruz

For full name: Juan Paolo dela Cruz dela Paz

Surnames: dela Paz

Given Names: Juan Paolo dela Cruz

Just make sure that this full name matches the info in the passport.. :-)
