Are you a health care worker seeking to perform such work in the United States but have not yet received certification from the Commission on Graduates of Foreign Nursing Schools or from an equivalent approved independent credentialing organization?

Filling out DS-160 right now. I am a Registered Nurse, and yes I plan to take NCLEX in a couple of years. I should say YES to this question, right? And explain that I am a Registered Nurse here in the Philippines and that I plan to take NCLEX in the future?

EVIDENCE OF SUPPORT.

You should be able to demonstrate that you will not become a public chargeor be a burden on the U.S. taxpayers for financial support. A completed I-134 Affidavit of Support Form with original signature will be useful to the consular officer to evaluate your petitioner’s ability to be financially responsible for you. You should also submit your petitioner’s most recent U.S. Federal income tax return (Form 1040) and wage statements (Form W-2). Employment letters stating salaries and banks statements may be included to substantiate the I-134.
My fiance works at Lowes and he asked HR for an employment letter as instructed in the I-134 instructions but their HR said they don't provide that, instead they can direct you to this website ---> www.theworknumber.com/verifier, give them Employer Code for Lowe’s: 11116, Employee's Social Security Number & a PIN. Will it be OK if I just submit his 2013 ITR, W2 and pay stubs? I think the earliest available interview schedule is March so he still has enough time to send me his 2013 ITR.

No problem with answering YES to the question, the DS-160 is designed with other visa types in mind which is why the question is there.

As for the employer letter, it is not required by the embassy, just one of those things you can include if you so choose, but don't stress if you don't not have one. What is required is the most recent tax return and W-2. If the income is adequate all is good! :thumbs: Relax.

Done with filling up the DS-160 too and i'm also a registered nurse here in the philippines. for that question, i answered NO even though i have a PLAN on taking nclex. Because for me, answering YES might be misinterpreted as my main reason of coming in the US well in fact, i just want to be with my fiance and apply to ANY job available.
