Asking here because the sample form wasn't clear and we just want to double check before sending anything in.

In Part 1, Family Name, since we're now married, would she use my last name as that's what's now on the marriage certificate?

In Part 3, Nonimmigrant Visa Number, which number specifically are they asking for and where is it found?

Question 1: if she is going to take your last name, yes she need fil out the form with your last name.

Question 2: maybe you can find that number in the visa k1 Print on her passport , just check to see if you see a number.

If she changed her name when she married you, then she now goes by that new name and all forms should be filled out in the new married name. Put maiden name only where it asks for maiden or other names used.
