Hello, my Brazilian fiancee wants to keep her entire name in addition to her new soon to be married name. Here is an example:

Mary(first name) Smith(maiden name) Family name (Jones) new married name(Sullivan)....so it will be Mary Smith Jones Sullivan!!!

My question is what exactly should I put in the other names used questions for the G-325a and I-765 forms? Would I leave it blank since she is not replacing 1 last name for another? We are basically only adding my last name to her full name?

Also another quick question....for middle name section on all those forms during adjustment of status I guess I would include her maiden name and her father's family name? Then for last name put her new married name which is of course my last name? Correct?

I wish I could avoid all this drama (worried that a USCIS officer makes a mistake) but its a brazilian cultural thing that I have to support her on

You should leave it blank. By 'other names' they mean in case she had had a previous name - in the event of a name change or something similar prior to becoming Mary Smith Jones...
