Not quite. It is still alive, although comatose, until marriage, the I-94 expires, or it itself expires (6 months from issue) - whichever comes first.

..... your K1 has expired it is a once entry visa, so as soon as you enter the USA it has expired....

How so not quite.... yes the K1 is valid for 6 months, so the holder has 6 months from the date of issue to enter the USA..... yes up until marriage if the K1 holder had to leave they could go back to the embassy and try for and extention. but the original K1 is now used up it can no longer be used for anything other than asking for an extention back in the persons home country... the I-94 is the item that is now the important one... if the marriage does not take place then the person must leave by the date on the i-94.... the date on the I-94 is also the date they must be married before....

You are correct that it's a new, re-issued visa that would get the person back into the USA.

Your first post made it sound as if a K1 visa has no remaining use after the one entry. But is does have a remaining use - in the situation of a departure before marriage and before the 90-day allowed stay has expired, in that it can be re-issued and a second entry can be made.

If the OP had been asking about extending the K1 then my original post would have contained that information but as the OP was asking about what date to put on the form I felt it was not prudent to include information that was not needed...
