I am thinking her alien number would be on the NOA2 it starts with an "A" it should be above her name

If you do not have any of the paperwork from your previously petitioned ex, then you can leave the A# blank. If you cannot remember the exact date you filed, then use just the month and year.

Also, is the name they are asking for the ex-fiance's original name or the one after marriage?

It is not really clear, but if they received a green card after you married, then the A# would be linked to their married name used for the green card. So, use that. You could also use the maiden name I would think, so either way should be acceptable.
