My husband is confused whether to put my Maiden Name in the Spouse's last name field. Does he has to put our last name of my Maiden name? thanks!!!!

well Im a man so your situation doesn't apply to me, but for my wife I did put her birth maiden name and in the interview I told the officer that we have the last name. So if you are the applicant and you have a green card, put your name as in the green card. Good luck.

no. you're right. i need a man's answer. my husband doesn't know what to put in the spouses last name under the marriage history category. so i will tell him to put my maiden name right?

Put your maiden name. USCIS knows that your name is different because of marriage and they have a copy of the marriage certificate.

thanks a lot. another question: my husband has a regular job and per diem job. does he need to include per diem also? but after doing per diem in a year the same company put him in a regular position. which date should be put. the regularization or per diem?

and also do I need to put N/A to some field that doesn't have answers?
