If I'm adjusting from B2 visa, and filing the i-131 together with my i-485, do I need to attach an explanation letter? If so, how should I write it?

You don't need to write an explanation, just a cover letter. I-485 can be filed along with I-131 and I-765. If you file both those forms with the I-485, there no fee for those applications individually, just $1,070.00 overall. Makes sense to do it all in one lump as opposed to paying the extra fees and file separately.

I made an overall cover letter for my attire package, then separate cover letter for the I-485 and included those two forms, I-864, and I-693 Medical exam. Then a cover letter for my I-130 petition.
