what do i need to send to the USCIS regarding my medical information for the Adjustment of status? I have had all the medical shots and my medical was done in September of 2013. Do I need to send my medical information or do i need to see a doctor for medical clearance?

He should have gotten a white envelope that has DS-3025 form, it should have been a copy and he should have received an updated shot record card??? If he does not have the DS-3025, then you will have to make an appointment with a Civil Surgeon to transcribe his shots, (if you have his shot record) Otherwise, you will have to complete the entire medical all over again including his shots, and mail it in with AOS.
