Hi, everyone. We're applying for the K-1 and want to get a head start on gathering the documentation.

1) I cannot find what information the Paris Embassy requests on the checklist. Does anyone have a copy of this?

2) Does her birth certificate and police record need to be translated into English by a sworn translator?

3) Does her birth certificate and police record need to have an apostile?

4) In terms of her medical appointment, is she required to visit the two doctors in Paris for the xray and bloodwork, or can she go to her own doctor?

Two updates:

I found an online copy of the Instructions for Immigrant Visa Applicants sent in Packet 4:

However, the Medical Appointment Information document that is sent in Packet 4 does not load fully:

Anyone have a copy of this?!?

I cannot open any of the links you posted.

I should get the mail from the embassy in the next few weeks, I'll have the detailed and updated checklist then and can share it with you.

If she is going to the Paris embassy, as long as her birth certificate and police record are in french or english, it's fine, (documents in any other language would have to be accompanied by an english certified translation). From what I read, she needs to get the "copie integrale d'acte de naissance" as her birth certificate, but I didn't find any info about the apostille, so probably not?

And for the medical, she has to go to to Paris with one of the two approved doctors. http://travel.state.gov/content/dam/visas/Medical/PRS%20-%20Paris.pdf
