I didn't pass an I693 as my medical is within the 1 year time frame that USCIS has established saying that if you are a recipient of a K1 visa then you only have to pass the vaccination supplement ( found in Initial evidence 7C of USCIS instruction). I didn't get an RFE because of that.

If the medical is over a year then you need to go see a civil surgeon and have him sign I693 and the civil surgeon is prolly going to put you on another medical exam. There's a new form of the I693 also. Here is a quoted statement from USCIS:

"Medical examinations completed on or after January 1, 2012, must be reported using the 10/11/11 version of Form I-693. If a medical examination completed on or after January 1, 2012, is submitted using an outdated form, USCIS will reject the form, and require the applicant to return to the civil surgeon in order to complete and resubmit the current version of Form I-693.

USCIS requires most applicants seeking to adjust to lawful permanent resident status to submit the results of a medical examination by a designated civil surgeon on Form I-693, which indicates whether an applicant is or is not inadmissible to the United States on public health grounds.

Note: The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status, but provide sufficient time for the performance of laboratory testing or additional testing required under CDC's Technical Instructions."

The K-1 medical had to have taken place within a year before filing for AOS. If was done less than a year ago, then you do not need another. If it was done over a year ago, then you need a new one done.
