DS-160 questions (Ukraine)

Guys,

I was checking over VJ the DS-160 form, and as always we have issues with it, so my questions are:

1. Name Provided: - do we put according to Ukrainian travel document the name (where is written name and last name) or we need to indicate our Patronymic (that so to say will stand for Middle name?) in some forums people say to put Name Patronymic Last name, some say do as in travel document.

2. Name in native alphabet: - do we put Name Last name or Name - Patronymic - Last name?

3. National identification number - it's logic to me that we should put the one provided to us in Ukraine (ідентифікаційний код) but again was reading some put Ukrainian passport number.....as they do for russian citizents for example...

4. Travel document number: - do we put with two letters that stand before the number or system does not permit to introduce letters, only digits?

5. Questions to which i do not have an answer, leave them blank or put "Does not apply"? (for example my dad is dead and have no idea where he was leaving...) than they ask about US Social Security number etc...which i do not have ......so state "Does not apply" or "leave blank"?

6. Country authority issued travel document: - in Ukrainian passports we have Authority issued and its marked by 4 digists.......put number or City and Region where it was issued?.

7. Have you traveled for the last 5 years: - I been to lots of places, do i need to name them all?

8. Provide list of languages you speak: - I speak 6 languages fluently.....do i name them all?

9. Do you have document that state you recieved vaccinations according to the US law? - I assume yes, cos they will give vaccination list during the medical, right?

10. Do you have immidiate relatives in the USA? - put fiance i assume?

11. Do you have other relatives in the USA? - I have grandma but we do not keep in touch....what to put?

Name as written on the passport.

Usually last name first, first name last.

Try it and see.

Then his status is Deceased and Place of Residence is Unknown

than they ask about US Social Security number etc...which i do not have ......so state "Does not apply" or "leave blank"?

Don't leave Blank. Your answer would be None.

Your call.

Yes.

Yes plus he may have something from his doctor or from when he was a kid.

Your fiance is not your relative. None is your answer.

Grandma is here" then Grandma it is.

Didn't fill out the DS-160 for fiancee (we did it before they switched to the DS-160), but did fill it out for some people for tourist visas.

Like passport

2. Name in native alphabet: - do we put Name Last name or Name - Patronymic - Last name?

Last name, First name - patronymic is not middle name, so you don't have to list it (although when we went with fiancee to interview, the Ukrainian that took our documents told us to put patronymic into middle name field on the paper form we had - this was before DS-160, but that's the only form where we did that, and they do not list the patronymic name on the visa anyway). I don't think it matters really if you list it or not.

3. National identification number - it's logic to me that we should put the one provided to us in Ukraine (ідентифікаційний код) but again was reading some put Ukrainian passport number.....as they do for russian citizents for example...

I put in internal Ukrainian passport number as the ID number, it was fine

4. Travel document number: - do we put with two letters that stand before the number or system does not permit to introduce letters, only digits?

Put the 2 letters

You can just put in Ukraine, or the city

Yes

Yes, they should fill out the vaccination sheet at the medical (bring anything you have from local clinic to the medical, so they can copy from it)

No, fiance is not a relative

11. Do you have other relatives in the USA? - I have grandma but we do not keep in touch....what to put?

Put grandma

There is one more thing, on US embassy page they have K'1 instructions, and say to schedule interview on ustraveldocs.com, and still there is caec web.......so i am lost......I file Ds-160 on caec but schedule interview on ustraveldocs or what????? that´s a mass........

There is one more thing, on US embassy page they have K'1 instructions, and say to schedule interview on ustraveldocs.com, and still there is caec web.......so i am lost......I file Ds-160 on caec but schedule interview on ustraveldocs or what????? that´s a mass........

Yes, schedule interview on ustraveldocs.

Also get the payment slip on ustraveldocs that you then take to Raifaisen Bank Aval to pay.

To schedule the interview you will need the receipt number from the payment that you get at the bank, and also the receipt number from the filled out DS-160 that you'll get at the end after submitting it.

For medical you just deal straight with the clinic (call them to schedule medical, and you pay them directly when there), the only thing you need is the KEV case number (preferably written on some official looking paper, they sometimes ask for it - the letter from the NVC is good).

That's the site I used multiple times for multiple people. Worked fine. Try in a different browser if it doesn't work, or try later, maybe there's something wrong with it temporarily.
